Megan Feighny Yoga + Pilates, LLC
Liability Release for Adult and/or Child/Children
I, individually and as the parent/guardian of the minor child/children identified below (if applicable) who may be participating in yoga/pilates classes offered by Megan Feighny Yoga + Pilates, LLC, hereby agree to the following:
1. I acknowledge that I am participating in classes or services which will include receiving information and instruction about yoga and health. I recognize that yoga and pilates require physical exertion, which may be strenuous and may cause physical injury to myself and/or my child/children and I am fully aware of the risks and hazards involved.
2. I understand that it is my responsibility to consult with my health care provider and my child/children’s health care provider prior to participating in any physical fitness program,including yoga and pilates. I represent and warrant that neither myself or my child/children have any medical condition that would preclude my participation and/or my child/children’s participation in physical fitness activities.
3. I also understand the contagious nature of the coronavirus/Covid-19 and that my instructor cannot guarantee that I as well as my child/children will not become infected.
4. In consideration of being permitted to participate in the yoga/pilates classes, I agree to assume full responsibility for any risks, injuries or damages, known and unknown, including possible exposure to Covid-19, which may occur as a result of participating in the program.
5. In further consideration of being permitted to participate in the yoga/pilates classes, I, as well as my heirs and representatives, knowingly, voluntarily, and expressly waive and release any claim I may have against Megan Feighny Yoga + Pilates, LLC, Megan Feighny, any instructor, the business owner, or the owner’s employees and agents for injuries or damages, including possible exposure to Covid-19, that either I and/or my child/children may be exposed to or sustain as a result of participating in classes or workshops conducted by Megan Feighny Yoga +Pilates, LLC
6. I grant to Megan Feighny Yoga, its representatives and employees the right to take photographs and videos of me, my property, and/or my child(ren) in connection with Megan Feighny Yoga. I authorize Megan Feighny Yoga, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that Megan Feighny Yoga may use such photographs and videos of me with or without my name and for any lawful purpose, including but not limited to such purposes as publicity, illustration, advertising, and Web content. I hereby waive the right to receive any payment for signing this release and waive the right to receive any payment for Megan Feighny Yoga’s use of any of the material described above for any of the purposes authorized by this release.
